The Pharmacy Team is a crucial part of our multidisciplinary approach and is responsible for overall medicines management. Their key role is to ensure you are on the right medication and the safest combination of treatments for your conditions.
The Review Process
- Reviews are often conducted in-person with you, but sometimes the team will review your records without you being present.
- In either case, you will always be notified of any changes made to your medication.
- When your medication review is due, you will be contacted by our Reception Team to arrange an appointment. Please ensure you attend when invited to keep your medications up-to-date and safe.
